Our form was working and now it is disabled and won't let us post the link

It says it's enabled, but says form not found.

The account where the above indicated form is built, is autosuspended by our anti phishing filter, because your form All Star Packet - Credit Card Authorization Form  is asking for credit card information in plain text fields.
