porquê não consigo acessar minha conta?

Perdi acesso a minha conta, a mesma parece bloqueada, não consigo nem fazer UPGRADE na mesma necessidade de ajuda urgente

Your account (pfsantos1) was auto-suspended by our Phishing detector.  That's why you can't login to your account. I checked your forms and I noticed that the form (http://www.jotformz.com/form/61065669180661) has a high-level score of phishing. This is because there is a label (GERADOR) GENERATOR on the form.
