How do I show Headers and Text fields in Encrypted Forms?

    I have an encrypted form that I need to show headers and text boxes - particularly text boxes that give permission or medical release information.  All that shows up when I print is a name and their signature.  This will not do me any good if I have to take a kid to the hospital.  I need to have the statement that they are agreeing to printed out.  I thought printing a PDF would help, but I can't seem to print it except from my My Forms Inbox area.  My emails are encrypted, which is fine, as I just go to the website to print it.

    If I understand your question correctly, you are printing the submissions from your form's submission page. You want to include headers and text fields also in the print. Is that correct?
