I did check your account "A001" and did not find any issue with that. It seems you have one another account with username "brian_chung" which is suspended by our anti-phishing system.
I did check your form "Clone of Password protected form" and it seems you have added conditions to show the reset of the form based on the text entry in "Key" field. I would suggest you to please edit your form title and remove the password text.
