My form is not shown anymore

I have embedded two forms on my website, but they are not show anymore.

When I try this link, it says the form is not found...

What happened?

We need this form urgently, please help us out.

I'm having the same problem - have used this form for years but it has suddenly disappeared from my website. I also need my customers to have access to my form urgently. I submitted a ticket privately but have not gotten a response - please help!
