How can my form be a PDF "photo" of the document rather than only the fields?

I have a form that is really a contract.  Significant text is prior to only four fields: name, email, birthdate, signature. The PDF shows the four fields but not the contract itself.....which won't work for me.

I hope you can help me today.  I really need them to get a copy of the contract they are signing.  Any suggestions??

Please try to enable the "Show Headers and Texts" option in the Submission page. Here's how:

1. In My Forms page, select the form and then click the "Submissions" button.

2. Click the "Options" icon and then enable the "Show Headers and Texts". After that, click the "Options" icon again.
