i used a form last year. I updated it this year changing the dates for the fields. My recipients are receiving the changes with no problem. However, when I receive their responses, they still contain the old info.

I cloned your form to test it and the dates in the email notification match the dates in the form:
I also did not see an autoresponder email so I am actually not sure how the users filling out the form are receiving a copy.
