Why my account is suspended?

    Hi, I created a form for an educational non profit and keep getting a form disabled error. Am I doing something wrong?

    Your account was suspended because you were collecting credit card information which is against our Terms of Use.

    I removed all those fields from your form, please avoid putting them back or your account will be suspended permanently.
