What should I do? I cannot login to my account (login: firstname.lastname@example.org) and also my forms are not available.

We apologize for the inconvenience your account is active again. It was suspended by our phishing system due to a form that was flagged as a phishing form because it is created in Polish language. I have white-listed it to avoid this from happening again.
The phishing system is not updated to scan form made in Polish language, so I would suggest you to translate your forms to English to avoid this from happening again: https://www.jotform.com/help/298-How-to-Make-your-Forms-Multilingual
