Hello,
I am trying to create a form similar to this Jotform: http://form.jotformpro.com/form/50909369094970? with the only difference that I would like to have 3 input values/3 questions that the user selects (i.e. Date of Birth, Gender, and Marital Status). Depending on their selections 3 different price packages would display (Low, Medium, and high). The answers are already predefined and I already know the answers for each possible option.
Can this be done? Thank you very much.

Yes, you can achieve that by using Conditions.
Complete guide: Smart-Forms-Using-Conditional-Logic
This basically allows you to craft IF/THEN statements that will manipulate other fields based on user selection. There are plenty of options to choose from (show, hide, update, calculate, etc.) when a condition is triggered, so I'll just list some related guides below that might help.
