My form is enabled but when I publish it is says the form is disabled.

    I modified a previously disabled form that I used to use to collect credit card details.  Since then I learned it was illegal to do that so instead I have created a form that collects pertinent details but not the actual credit card information.  It is still not letting me use the form even if I cloned it.  I deleted the original credit card form.  Can someone please turn this back on so I don't have to recreate this entire form that took me several hours to create?

    I checked your account and I noticed one of your forms which is (http://www.jotform.us/form/70777018729163) has labels which are "Payments" and "Billing" which has a high-level score of phishing. Please note that when these labels are used in the form, the Anti-Phishing Tool will detect and will automatically suspend your account. And it also will make your forms disabled.

    If you want to receive payments from the users of your form, you must use the Payment Processing Gateways available in JotForm. Please see this link: https://apps.jotform.com/category/payment_processing.

    I have now removed the said fields and re-activated your account, please refrain from doing this so that your account will not be suspended again.
