I accidentally deleted a form how do I get it back from Trash??

I tried to publish a form but accidentally managed to click to send it to trash, I then pressed undo yet it failed to do so. I need this form, it cannot be deleted. Please help.

Deleted forms can be restored from the trash.  Here is how to do so:
