Is there a way to get the form once submitted with the text and questions?

    While viewing your submissions, there is an option to include headers and text in your submissions:

    Here is a bit more in depth guide on having your printed/PDF form look like the full contract:

    Once that is enabled, headers and text from the form will be included in your submission data.  If you would like to email a copy, use the "PDF Attachment" option in your notification settings:

    The PDF copy would include form headers and text.
