What is wrong with my form?

I am getting an error message when I try and update this form.

All it is showing is a black screen and saying "Oops! Something went wrong and We couldn't initialize a part of your form correctly." "Reload Page" but when I hit the reload page button it doesn't do anything.

We bug that caused forms to not be editable in V4.  It has been fixed and you can now edit forms once again.  Apologies for the down time.
