Why I can not see my form?

I created few form here, and put the code on my website.

It worked few days ago,but now it do not work.

Do you know what is the problem? my account problem or other things? 

I can see that you're a sub-account user. We had an issue about sub-users unable to view the shared forms but it is fixed already. I checked your account and I can see the shared folders now. See image below:

If you're referring to the form not displaying in your website, please try re-embedding the form using iFrame method. Here's a guide: Getting-the-form-iFrame-code.
