Calculation-based responses

    Hello,

We are looking to create a form that consists of 10 questions. Each one is a yes / no question, with yes = 1, no = 0.

Upon completion, we would love to send an automatic email with content that depend on a calculation. For example, if the sum is 4 and below, they get one response, if it is 5 and above, then they get another.

Alternatively, do you have a visual way that the user can see with a gauge or some meter? (green, yellow, red kinda thing)

Is that possible to do with your tool?

Your set up should be possible.  You would assign your options calculation values:

    https://www.jotform.com/help/301-How-to-Assign-Calculation-Value

Then total up those values in a calculation field:

    https://www.jotform.com/help/259-How-to-perform-calculation-in-the-form

Set up your different autoresponder emails:

    https://www.jotform.com/help/26-Creating-a-Form-Autoresponder

Then send those emails conditionally based on the total in the calculation field:

I am not sure sure how you would like to display the gauge.  If you wouldn't mind elaborating, we will see what we can do.

    Thank you!
I will check the links. Regarding the gauge, I would like to show in any
way visual to the user the result ... any type of meter or gauge that has
    green / yellow / red...
    Something like:

For the gauge, you could add three different images to your form, then conditionally show those images based on the score:

    https://www.jotform.com/help/316-How-to-Show-or-Hide-Fields-Base-on-User-s-Answer

Something along the lines of, if score less than 10 show image one, if score greater than 9 less than 20 show image 2, etc.  It wouldn't be a gauge that fills up based on the score, but the form would have the appearance of it doing so.
