Is your company currently insured ? (if they check the box that says yes, it will trigger another question which is below)
who is your current carrier ?

You can indeed show different fields in the form based on previous input. This can be done using a show/hide condition:

For your example, you would create a condition like this:
IF Is your company currently insured IS EQUAL TO Yes SHOW who is your current carrier
