"Form not found. This form is disabled"

A form I've been working on has stopped working. I get the error message "Form not found. This form is disabled". I've tried disabling and enabling it again but it isn't working.

The message is because the account where this form was built is suspended because are requesting username and password in plain text fields.

Gathering this type of data in plain text fields will violate our Terms of use, as this is only allowed through encryption along database administration; and our forms do not provide this type of method.

We have removed the guilty fields on this form and reactivate the account. You must refrain from doing a new form like that to prevent a potential suspension of your account.
