How to import a fillable PDF form?

    Hi I am trying to upload a fillable PDF and it is not working and I am curious as to what I am doing wrong or if this is the best solution to my problem. I work for the US Polo Association and we are trying to make it easier for our Umpires to submit their paperwork than an email or in person, we think providing a link to a jotform would be perfect. The only down side we are having is that we want to be able to have the document do the basic math for them. Thre seem to be a few widgets which might do this in jotform but they wont let me edit the top of the colums. So we thought uploading a fillable PDF might be the best way because we can program it to do the Math and then upload it and it would work. Unfortunatly we are having some problems uploading. Please let me know if this is the best solution and ow to go about fixing this.

    Is that fillable PDF form from Adobe FormsCentral? If not, the PDF import option will not work, since it was created to work with Adobe FormsCentral Fillable PDF forms only.
