    How do I get the notification email updated to include new questions that have been added to a form?

    You could add a new field to the email notification by editing the email: 

    And clicking on the "Form Fields" option to get the current fields of your form, clicking on it will add it to the email template: 

    You could also delete your email notification and add it again to get the updated fields.
