I have deleted and recreated my notification email and autoresponder email multiple times and whenever I log back in there is extra code at the top of the email and missing fields. I have tried manually changing the form, but it will still change and leave out fields that I had put in the email.

Also I will attach screen shots with a newly created email notification and what it looks like after I refresh the page. These show the extra code at the top of the email and also the missing fields at the bottom of the email.
Please advise on how to fix this issue.

I cloned your form and replace add another email notification and the extra codes aren't appearing to the new email notification.
I have now cleared your form's cache. Please add again an email notification and see if this works now.
If the issue still persists, please also try clearing your browser's cache and to try to relog into your account, or start a fresh browsing session.
