Conditional logic not working

I set up some conditional logic that used to work over the last few weeks (see conditions 96-103). No they don't anymore. No idea why. Please help me figure this out! Thanks.

There was an error on our end that caused calculations to fail.  Please accept our sincerest apologies.  This is now fixed and calculations should be working once again.  If the issues persists edit the form and save it.  That will remove the bug.
