Why am I getting "Form not found" error?

    Hi,My account is cwalkercfli21 and all of my forms are down.  Is there a service outage? I am on an unpaid account and only have 5 forms. I am also under my monthly limit. Any assistance would be appreciated : )

    Hi,

    I have checked your account and it is seen that it was auto suspended by our system because of phishing issue. I have checked your form 62906479302156 and I noticed that you are collecting driver license, passport and State Issued Identification Card Information via Image Upload.

    Please look at our Terms of Use. You cannot gather sensitive information from the submitters. If you collect these data, your account was suspended by our system automatically.
