I checked your form (http://www.jotform.us/form/70817003993155) and I noticed that the Hover Texts on your form with line breaks appear just fine.
Are you still having issues on Hover Text on your form?
As per checking your form, I also noticed that your account was auto-suspended by our Anti-Phishing Tool and your form is disabled. I see that there are fields that ask Username on your form. Please note that asking such info to the users is against our Terms of Use. If you want your account to be re-activated, please remove those fields on your form.
