Could you check your records.
I feel I have been erroneously Suspended.
I have been subscribed to your service on the for over 3 years and I have not for once abused the service.
Purpose of my usage: To Generate Registration Code for Applicant of a current Project.
If you have asked for username or password in one of your forms, please describe the reason: I have NEVER requested for Username and Password on any of my past or current forms. You could please verify in your record.
Provide the URL of your web site where you use these form: www.DejiKing.com
Kindly have this rectified ASAP as this project is solely dependent on the form.
It is most certain that we would require a paid package when we have larger projects in the sooner or later future

All those words trigger our anti-phishing system. I removed them from your form so I could re-activate your account. If you could put some other wording there so it can work for you that would be great.
