How to hide fields on my form based on conditions?

i am trying to obtain one years address history in my form, so a 2nd and 3rd address box will appear based on the length of time at an address (they would remain hidden if anyone has lived at an address for more than 1 year), but i cannot get this to work. any ideas? 

You may have multiple address fields on the form and show/hide them based on the selection of a number field which may be the question something like Number of addresses that you lived in in the year?
