How to hide fields on my form based on conditions?

    i am trying to obtain one years address history in my form, so a 2nd and 3rd address box will appear based on the length of time at an address (they would remain hidden if anyone has lived at an address for more than 1 year), but i cannot get this to work. any ideas?

    You may have multiple address fields on the form and show/hide them based on the selection of a number field which may be the question something like Number of addresses that you lived in in the year?     Alternatively, you may also consider using the Configurable list widget that allows adding multiple field types and multiple rows. Please refer to the guide below that can help you with configuring Configurable list on your form.
