I have not received the filled forms by some of the participants

People have filled and submitted the forms. I have not received it. I would like to know, why are those responses missing. Each response is important to me. At least 10  filled forms are missing.

Please help me, I have paid money so that I can get maximum responses. I can't afford to miss it.
