Header is not showing up

The header on my form is not showing up. It's on the "Build" screen, but not on the url site. It very quickly appears and then disappears.

I can see an image at the top of your form if that's what you're referring to.

To double check, I cloned your form and saw no difference between the Form Builder and Preview. They both have that image at the top.
