We have an issue with a submission that is missing a bunch of data. Particularly, the one with the name Marlon Hernandez. If you see screenshot 1 below, the submission only shows ID, Submission Date & IP, whereas in the Gmail notification in Screenshot 2, it shows with the complete data.

I've highlighted the IP address, that's how we knew it's from the same person.
Any help would be greatly appreciated.

We would like to apologize for any inconvenience. To be honest, I can only think of one reason why that particular submission is emptied but and the others before it and after it is not.
I am not 100% sure at this time but there is a possibility that the submission was edited, was cleared, then resubmitted. And since when I checked the form, none of your email notification and autoresponder has its "Send on Edit" option enabled, there is no email (in the email history page of your account) that would show that the submission was emptied.
Please allow me some time as I would like to make a few test. May I ask you to create a test form (a simple one) on your account where we can make test submissions, please?
