Account Autosuspended

    I noticed the forms on my website say form not found.  I was not told that my forms would be disabled for any reason.

    I haven't signed on to JotForm in years so I was trying to guess my password.  I also let Dashlane try.  Now you have autosuspended my account.  Please reactivate it so I can reset my password and build some new forms.  It's costing me business every day your forms don't work.

    I have checked your account to verify the reason why it got suspended and found the form entitled, "2017 Preventive Maintenance Programs", having credit card number and expiration date fields in it.

    Please be informed that we have an anti-phishing tool that scans every form in every account 24/7. Any forms found or suspected of violating our terms (Terms-of-Use) will result in automatic suspension of the account that owns it.

    In order for me to help you reactivate your account, I have deleted the fields from the said form that caused the issue you're having. You should be able to access your account and forms again.
