Is it possible to put a hyperlink in an answer?
i.e. if someone selects Yes - go to www.linkgoeshere.com
Or, is the only option to create a text box with this link in it?
If so, how can I make it so that when this link is clicked, it opens in a new window?Page URL:

Yes. It is possible to use HTML anchor tag with the options so that you can link the option to your web site as shown below:
<a target="_blank" href="http://www.example.com" rel="nofollow noopener" >Option 1 </a>
The options will be displayed on the form with the link.
