Javascript errors on the page where my form is embedded

    Hi there. If you go to the following page where I have the form embedded, you can see in the console that there is a javascript error.

    Can you let me know what the problem is?

    I have examined the webpage you kindly provided, where your form is embedded, and I would recommend re-embedding the form using the iFrame method.
