I have tried submitting your form on a mobile device and see that the Thank you page is displayed after successful submission.
Are you still experiencing any issues with the form on mobile devices? Is the form embedded on any web page and having the issue? If so, please provide us with the web page URL where the form is embedded so that we can take a look and see what best we can do in this regard.

I have checked the web page and it looks like that you are using Wix form builder if I am not wrong. As I check the web page on a mobile device, I see that the form is displaying normally and was able to see that the form is scrolling. Though I am not able to find any issues with the form except for scrolling due to the less frame width than the form.
I have tried submitting the form on my mobile and was able to submit it normally.
In order to fix the scrolling, you may increase the height of the frame where you have embedded the form on the builder.
