How do I host the form I made?

i made a form using a theme and i want to publish that one page called ANTHEM PRE

i have bought two domain names that i want to use for this single page, how do i assign those domains to that single page form? 

the whole page with background image and form will all be hosted in my jotform account correct?

How do I assign those domains to that single page form? 

You need to add a CNAME and DNS record in order to use your own domains. Please check this user guide: https://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform

Yes, the background image is hosted on our servers. You can use it in your JotForm account without uploading it to your server.
