Could someone please remove a page on my form

I always have issues with my form whenever i try to delete a page. May somebody please remove page 16 of my form? Thank you.

As per your request, the page 16 was removed from your form. You can also do this by deleting the 'next&back' buttons on the previous page. This will automatically deletes the last page.
