How to insert a Likert-like scale in form?

    I would like to use a Likert scale that is similar to the image below.

    While the rating scale gives me the possible 1-7 choices, I am not sure how to make the labels change to my text choices, or have it scale horizontally to fit the layout width.

    Since there are over 30 questions of this sort, is there a way to get this done in one of the tools provided?

    If I understood you would like to have a scale for your questions, but I didn't understand how would you like to show it and where. So will the progress bar work for you?

    If you need to calculate some sort of score (it can be rating as well) for your questions we can do that by assigning a calculation value to each answer and then to calculate the final score (rating).
