Error in form submission

Hi, I am having trouble with this form - https://www.jotform.com/build/52083541285959. When submitting the form regardless of what is entered it wont always accept the form without playing around and marking or unmarking different fields. Even when all the required fields are put in. Also the payment information has not been coming in now for some time. What needs to be fixed here and how can we do that?

I cloned your form, and the issue was reproduced on my end too:

The issue occurs because of the "Shipping" product you have setup in your payment field. Please delete it, and create it again. This time, make sure to assign a value(0.1) in the price field:
