Background color

How can I change the background color, or add am image to the background.  I'd like to do something like this, but to have my background as black, the fields clients write in white, and the headers of each field in blue. I'm trying to make this form as congruent with my website as possible.  (www.davidenglandphotography.com)

In advance thank you for your assistance.

This and much more can be achieved using the Form Designer.

    Follow the steps provided below

    Step 1  Access Form Designer

Wonderful. Thank you for the screen shots. That was really helpful. I've
completed my form, and I've gone through the steps to embed your form into
a custom page on Zenfolio. I've followed their instructions, but the
embedded code from the from I built won't show up. Do I need to do
anything to make the form live so I can successfully embed the page I
created from you? I've attached screen shots to show you my progression
and what the end result looks like. In advance, thanks for your assistance.
