Setting up a multi-sport form with limit of submission.

My website is www.powerhouseclassic.com... It is a college recruiting site. It allows the player of the many different sports to complete the form and become a member. They are then allowed to send their created player profile to up to 10 colleges no more than 3 times. The colleges are to be listed in a drop down box. So I want to build a form that can cover each sport if selected and then process the payments. Each membership last 1 year.

I am not sure if I have understood your question correctly. Do you mean to say that you want to allow one user to submit form to 10 colleges and from each college, form can be submitted 3 times only. 

This seems to be a little complicated requirement but before we can help you we have few questions:

#1. Have you created a sample form with the question you would like in form?

#2. How many colleges you will be listing in form?
