When a member filled out the membership form, only the Submission Date and Signature is shown in the PDF version, and in our Google Spreadsheet, there is a line for the submission but the entire line is blank. Only the submission date and time and signatuer is shown. When I go to Edit Submission, the entire form is empty except for the signature.
Do you know how this person was able to submit the form when we have "Required" fields?

I cloned the form and tested it by submitting the form.
I could not replicate the issue at my end. The pdf attachment showed all the values along with the signature
Submitted data also showed all the values in submission.
