Is it possible to create anonymous submissions for a form?

Hi,

I'd like to create a form that employees can submit feedback to management anonymously. Is it possible to create a form that does not track the user who submitted?

As long as you do not request the information or mark it as required in the form, it will not be collected.  If you don't want to know the name of the employee who is submitting the form, don't add a name field.

The only potentially identifying mark for each submission would be the IP address of the form submitter.  Unless the computer they are using always has the same external IP address and those IP addresses were readily available, there would be no way of matching submissions to users.
