Is your PDF form a "Fillable FormsCentral PDF Form"?
Please note that this feature only works for fillable PDF forms from Adobe FormsCentral: https://www.jotform.com/help/340-How-to-Import-Adobe-FormsCentral-PDF-Forms-to-JotForm
Let us know if you have more questions.

Rachel_RomanReplied on March 30, 2017 at 3:45 PMNo, unfortunately, it is not. We are trying to figure out how to get it to be on Jot Form and fillable, rather than creating an entirely new form.
Thanks,

You would have to create the form from the scratch. We do not have a feature to import PDF forms that are not from Adobe FormsCentral.
