Submit button has disappeared from my form

I created this form, and applied conditions to the last question on pages 2 thru 11 to skip to page 12 if the answer is no. The last page has a box that must be answered yes, and a signature widget that is required.

I have cloned your JotForm and see that there is no Submit button on the form. It looks like that it has been removed accidentally. Please add the Submit button to the form so that the form should be working fine.

I thought I did not see that option in the list, but then I found it. The form was acting crazy because when I would try to delete the last page, it would delete the first page break instead. I'm guessing it happened while I was fighting with the form to do what I wanted.

When I found it, it worked.
