Can't verify email address or view/preview a form I created.

When trying to preview the for it say, "this form is disabled." When I try to verify my email address it says, This link is not valid!.

I checked your account(ManageWerks) and I noticed that you have a form (http://www.jotform.us/form/70886754134161) that asks for Social Security number. Please note that asking such info from the users of your form is against our Terms of Use and your account will be auto-suspended by our Anti-Phishing System.

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

I have now removed the guilty fields on your form and re-activated your account. Please refrain from doing this so that you won't be suspended again.
