I am trying to log into my account but when I do so it gives me an "auto suspend error" upon login. Please help me with this.
The problem is on my webpage listed below under the buttons "Get Started." I have attached a screen shot.

The form "Website Design (Internal Use)" violates our terms of use. The form is asking for login information such as username and password.
We can delete the form "Website Design (Internal Use)" and re-activate your account. We'll be needing your confirmation if that is okay with you.

Thank you for your co-operation. We have now deleted the guilty fields on the form and activated your account. The forms should be displayed normally. Please refrain from adding such fields to the forms to avoid any further suspensions on the account.
