Opened three forms to edit and forms broke

    Hi there.  I wanted to change a bit of text of my emails associated with a few forms.  When I did that, it broke my forms and I have no idea how to fix them.  I made no changes to the design of the forms at all, but now the forms don't display correctly on my websites: the form fields have changed design and size and the submit button disappeared.

    Take a look at this website: http://www.moldinspectionsciences.com/about-us/.  See the form on the sidebar?  This is how the form should look.

    Now, take look at this similar page: http://www.phoenix-mold-inspection.com/about-us/.  See how the form on the sidebar is broken?

    I need to return the forms to how they were before I simply opened them and they broke.  The forms that broke are the side inquiry forms for Phoenix, San Diego, and Austin.

    Can you tell me how to fix them or fix them?  Also, why did the forms break when I didn't change anything having to do with the design?

    Besides changing the look and size of the text boxes and hiding the submit button, there is also a white background behind the form...

    Again, I made no changes to the design.  I opened the Phoenix side bar inquiry form to make a change to an email and it broke the design.  So, I wanted to see what another of my side bar inquiry forms looked like hoping I could fix the Phoenix one, so I opened Austin side inquiry.  I made no changes and it broke that one too.  So, I tried to go to the old Jot Form interface hoping I could fix them and opened San Diego and it broke that one.

    Also, I checked your forms directly, but I don't see anything wrong with them.
