Field name not the same in email alert?

On my actual form, the field is clearly titled: Dismissal Time.

When the form is completed & submitted, that same field changes to: Arrival Time, thereby duplicating a previous field only on the "Submitted" version.  See Test below.

    Why?

May I know if you have edited or updated the form? Upon checking your account, I see that the email templates are now using "Dismissal Time" as the label, and the field tag seems to be the correct one:

    Notification Email:

    Autoresponder Email:

If you are still having problems with this, I suggest deleting these email alerts and creating a new one. The newly created email alert should automatically have the correct field labels and tags on its template.
