    Email Notification: Text not showing in form after completing the form.

    I created a form and sent the link to myself in an email.  The header and text boxes aren't displaying.  I read another help suggestion and checked the "show headers and text" box in preferences in My Forms/submissions.  The text is still not showing up.  Any suggestions?

    Unfortunately, the show headers and texts will work for PDF reports only, for email notifications, there's no way to include the text automatically. 

    If you would like to include it, then you would need to manually add the text to your form. Another way is to use the Narrative Fields widget, that will help you to display the text only and include it in the submissions.

    When you say manually, do you mean each time that I want to send it, I would have to do this, or is it a one time thing.  
    Also, is there a way to convert my form to publish as a pdf?Thanks

    No, you will need to add the text to the email template only once, then it will be included in all the emails you receive.
