The form field values will not clear after submission

When a respondor submits a form the next person to open the form can see the previous respondor's responses (including identifying information).  This problem has only recently arisen.

I have examined the form that the page URL you kindly provided, points to, and found that it has autofill enabled, which explains why the form field values are saved locally. Disabling this feature solves your problem.
