the form does not clear after submission.

After a form has been submitted the next person to open the form can see the previous person's responses including identifying information.  

I cloned your form (https://www.jotform.com/70474935096162) and I noticed that you have enabled the Autofill option in the Continue Forms Later feature. In this way, the next submitter will see the previous response of the submitter if they share the same browser. Enabling the Autofill feature will automatically save the form's field values locally (browser local storage) until the form is submitted. 

To be able for the next person not to see the previous data of the previous submitter, please disable the Autofill option from the Form Settings.

Settings > Form Settings > Continue Forms Later.
